Healthy Skin Care Habits for the New Year

For most of our Santa Barbara medical and day spa clients, skin care is an on-going “work in progress”. In essence, many people don’t wake up day after day with healthy-looking skin from doing nothing. Instead they “work” for it with regularly scheduled day and medical spa treatments and maintain their results by following a recommended skin care regimen at home. Not surprisingly, there is more to achieving healthy skin than facials and medical-grade products. Developing some of these healthy skin care “habits” will also help boost your glow!

Hands Off
Hands and nails can contaminate your skin. Commit to touching your face as seldom as possible throughout the day.

Eat from the Earth
Colorful vegetables and fruits are packed with vitamins and antioxidants that can repair, enhance, and nourish your skin from the inside out. Greasy or fatty foods tend to upset the natural balance of your body and can cause your skin – and overall health – to deteriorate.

Get Your Rest
“Beauty sleep” is a pretty accurate phrase. While you sleep, your metabolic rate lowers so you are not producing as many skin damaging free radicals. Your skin also focuses on repair (instead of protection as it does during the day) when you are asleep.

Aim to exfoliate your skin 2 to 3 times per week. This will encourage cell turnover (which decreases every day that we get older!) and will automatically brighten your complexion.

Use Sun Protection Year Round
Whether rain, clouds, or sunshine is in the forecast, get in the habit of applying (and re-applying after prolonged sun exposure, excess perspiration, or exercising) sun screen to your face, neck, chest, and hands.

Invest in Your Skin Care Routine
If there is one skin cleansing tool to invest in, it is the Clarisonic facial cleansing brush. The Clarisonic is Dr. Perkins’ #1 recommendation for your home care regimen and one of the best ways to maintain or achieve healthy skin. We carry the Clarisonic SMART Profile and the Clarisonic Mia 2.

Clarisonic Skincare: Santa Barbara Medical Spa Event Recap

Our “Summer Skincare” event provided us with so much information that we have decided to split the recap into three posts – each dedicated to the three topics of the evening – all of which are equally important.  The first topic is about the Clarisonic Skin Care System.

What is the Clarisonic Skin Cleansing System?

clarisonicThe Clarisonic skin cleansing system, or popularly known as the “Clarisonic brush”, uses a patented sonic frequency of more than 300 movements per second to gently, and thoroughly, cleanse your skin.  It removes six times more makeup and two times more dirt than cleansing with your hands alone.  The inside area of the brush oscillates ten degrees in either direction, moving with the skin’s elasticity, while the outer ring of bristles stay stationary and act as a splash guard.  The movement of the bristles makes this brush unique to all the other electronic skin brushes on the market, which use a rotary movement that tugs at the skin and can cause damage. Use the Clarisonic with any cleanser of your choice as long as it is non-abrasive.

What are the benefits of cleansing with the Clarisonic?

If you use the Clarisonic on a regular basis, once in the morning and once in the evening, you will see many benefits, including:

  • Clearer, smoother skin
  • More product absorption – your cleaner skin will absorb moisturizers, and other facial products, more thoroughly and evenly
  • Less blackheads because it removes dirt and sebum (oil)
  • Minimized appearance of pores

Clarisonic Brush Heads: Which is right for you?

There are five types of brush heads available for the face, and one for the body. The facial brush heads target different skin types:

  • Normal: perfect for normal skin, shaving prep and use on the neck and chest (this is the brush head on the grey plastic base)
  • Sensitive: for sensitive to normal skin types (this is the brush that comes with all Clarisonic units)
  • Delicate: for delicate or extra-sensitive skin (this brush has a light blue plastic base)
  • Deep Pore: use for cleansing on oily skin, large pores, and is gentle enough for daily use
  • Acne: extra-plush with bristle gradation to provide gentle cleansing for sensitive and acne prone skin

Clarisonic Settings for Optimal Cleansing

Depending on the model you buy, the Clarisonic has different speeds, or settings, ranging from a sensitive cleanse to a deeper cleanse.  The Clarisonic Mia (a popular unit for travel that comes with an international charger) has one speed, while the Mia 2 has two speeds.  The Clarisonic PRO has four speeds to meet all skin cleansing needs.

Tips for the Clarisonic

  • We learned last night from the Clarisonic rep a secret that has to be shared! Your brush can do double duty for spot treatment. Pop out the center of the brush, snap it on the Clarisonic and use this smaller brush head for the area between the nose and the cheeks and the nose itself.
  • Clean your brush head with antibacterial soap at least once per week
  • Use a permanent marker to write the date on the back of the brush head and remember to replace the brush every three months
  • For those of you who like to use the Clarisonic on your back, there is a back handle now available that attaches to the Clarisonic Pro

At Evolutions Medical Spa we use the Clarisonic brush as part of our facial services. Dr. Perkins and our staff highly recommend incorporating the Clarisonic cleansing system into you skincare routine to maintain beautiful skin in between facial services.  The Clarisonic system and brush head replacements are available for purchase at Evolutions.

Santa Barbara Skin Care Event at Evolutions Medical Spa Recap

Yesterday evening’s Skin Care 101 Event at Evolutions Medical Spa Santa Barbara was packed!

Guests indulged in complimentary Clarisonic and Vibraderm mini-treatments and Colorescience mineral make-up application. Santa Barbara cosmetic surgeon Dr. Terry Perkins educated clients on the importance of home skin care including essential steps and the quality of products. Recap follows:

Spa Procedures vs. Home Care: Which is More Important?
The answer may surprise you: both are equally important to achieve a desired result such as healthy, smooth skin that glows.
Routinely participating in spa procedures (facials with extractions, chemical peels, professional exfoliation with Vibraderm) contributes to about 50 – 60% of your desired end result while home care contributes the other 40 – 50% in helping you toward fabulous skin.
The take home message is that consistent use of professional home care products builds upon the work that is done on your skin at the spa.
High Quality Home Care: Pharmaceutical Grade Products vs. Mass Market Skin Care Products
Dr. Perkins highly recommends pharmaceutical grade skin care products because they have higher active ingredients which renders them more powerful. Mass market skin care products have lower active ingredients so that they can be used by a large audience without many problems. In mass market skin care, usually the packaging is half the cost of what you are buying.
The Five Essential Steps of a Comprehensive Skin Care System
Consistently incorporating these steps into your home care regimen is key to obtaining and maintaining your ideal complexion.
  1. Cleanse twice daily. Use the Clarisonic skin care brush when washing your face. The 1 minute sonic cycle gently removes dirt and sebum under your skin, up to 2x more than cleansing with your hands alone.
  2. Exfoliate two times per week. Chemically or physically exfoliating the skin helps to slough off the older skin cells and eventually creates more evenness to the surface of the skin.
  3. Antioxidize three times per week to decrease damage from free radicals that cause aging and possibly the beginning of skin cancer. Applying a topical antioxidant on your skin is much more powerful than taking something orally. For example, topical vitamin C (known as L-Ascorbic Acid) packs 50x more of a punch than would ingesting vitamin C. We are not discouraging you from eating food that is rich in antioxidants rather we encourage you to use a topical antioxidant along with eating plenty of fruits and vegetables! A powerful topical antioxidant moisturizer made by Revale contains CoffeeBerry® – derived from the fruit of the coffee plant – it has one of the highest antioxidant scores recorded. Revale is available at Evolutions.
  4. Retinoids – commonly known as “Retin-A” – are great to use two times per week. You may have heard of Retin A (or “tretinoin) traditionally used for acne. Research has found that retinoids also normalize cell differentiation along with decreasing the plugging of pores and helps to correct DNA damage which may help to keep skin cancer at bay.
  5. Sunscreen – this is the most essential step – and must be used daily, year round! Preferably the use of a physical block sunscreen is best as it deflects the UVA and UVB rays. Physical suncreens contain titanium dioxide and/or zinc oxide. At Evolutions we carry Obagi sun protectors (Skin Protection in SPF 35 and Sun Shield in SPF 50) and TiZo (in various SPFs).
Evolutions Medical Spa carries various medical grade skin care products that Dr. Perkins has hand-picked because of their therapeutic value and high level of satisfaction by clients. We also offer a complimentary skin care consultation by filling out this form or calling for an appointment at 805.687.0212.

Santa Barbara Medical Spa Product Focus: Revale Skin Care

Antioxidant-rich foods, such as colorful fruits and veggies, are considered super-food because they help strengthen the immune system, staving off a cold, the flu or infections, and some people even believe they play a big part in warding off chronic diseases (such as cancer, arthritis and more) because they contain high sources of vitamins, minerals and nutrients that protect and repair cells from free radical damage. These foods can really make a big impact inside your body.

What about fighting free radical damage on the outside of your body? Free radical damage shows up on our skin as sun damage (hyperpigmentation), fine lines and wrinkles and skin cancer. Are you thinking that if only there was a way to apply antioxidants to the skin to lessen and/or prevent free radical damage . . . Well, there is a way to do that! Santa Barbara cosmetic surgeon, and founder of Evolutions, Dr. Terry Perkins, recommends Revale skin care products as the “top of the line” antioxidants-for-your-skin products on the market today.
Revale products are formulated with CoffeeBerry® extract derived from the coffee plant (Coffea arabica) and has one of the highest antioxidant scores ever recorded. Topical application of this “active” antioxidant will help repair prior environmental damage as well as decrease future damage.
An easy 3-step system that is gentle and suitable for all skin types (and won’t clog pores) is available at Evolutions Medical Spa Santa Barbara:
  • RevaleSkin Facial Cleanser: Purifies and clarifies.
  • RevaleSkin Day Cream with SPF 15: Smooths and softens.
  • RevaleSkin Night Cream: Revitalizes.
We can all benefit from a good dose of antioxidants daily on the inside and the outside!

Evolutions Medical Spa’s Event Recap: Tightening and Lightening Your Skin, Without Surgery

Last week, Evolutions Medical Spa and Santa Barbara cosmetic surgeon Dr. Terry Perkins hosted an event that is a popular topic amongst many of our clients – skin tightening and skin lightening using advanced technology that gives results, without surgery. Read on for a full recap:

Part 1: Skin Tightening – Treating Skin Laxity

What is Skin Laxity?

Skin laxity, or “loose” skin, begins as a natural part of aging, due to damage and loss of collagen and elastin, but also related to sun damage.

Treating Skin Laxity

There are surgical options to successfully treat skin laxity such as:

  • Deep Peels: Chemabrasion, a deep peel that superficially destroys the epidermis and dermis, done under general anesthesia.

There are also successful non-surgical treatments:

    • Light-based treatment: such as the FotoFacial RF (photo rejuvenation), the safest and most effective intense pulsed light (IPL)-based treatment today.

  • Radiofrequency treatment: such as Sublative Rejuvenation, the newest skin resurfacing device with less downtime and discomfort.

How Do We Treat Skin Laxity Non-Surgically?

We have found that sustained heating of the dermis (the cells below the skin and the epidermis) has both short term and long term effects: Immediately it stimulates collagen contraction, which helps to tighten the skin while it also promotes long term stimulation of collagen remodeling – maintaining a strong collagen “support system” gives the skin strength and resilience.

Who Can Be Treated Non-Surgically for Skin Laxity?

    • Clients that have the best results have a minimum amount of excess fat in the area of concern and who have skin that is neither very thin or very thick.

    • Clients can be anywhere from 30 to 80+ years.

    • When treating skin laxity non-surgically, clients must acknowledge that the treatments will turn back the clock a few years, not decades!

  • Someone who is not ready for surgery, but ready to see a difference in their skin, is a great candidate.

The Titan Procedure: A Non-Surgical Skin Laxity Treatment

Titan uses an infrared light source to heat the dermis well below the skin’s surface. This heating contracts existing collagen and stimulates growth of new collagen. Although a single treatment may show a change in your skin, a series of two to four treatments is recommended for optimum results. Downtime is minimal, with maybe some mild swelling and redness for a few hours post-treatment. Clients have been successfully treated on many areas of the body. The most common areas treated are the jawline, under the chin, and the abdomen.

ReFirme™: A Non-Surgical Skin Laxity Treatment

ReFirme™ combines Bi-Polar Radio Frequency and Light energies to heat the dermal tissue within the targeted treatment area. Using significantly less optical energy than lasers and IPL’s, it ensures enhanced safety and a virtually painless procedure. It helps skin laxity, sagginess, wrinkles and textural irregularities and most patients see gradual and cumulative results throughout the treatment regimen. Common treated areas are the face, neck, abdomen, upper arms and knees.

Total Tightening: The Best of Both Worlds for Treatment of Skin Laxity

Total Tightening is an exclusive protocol designed by Dr. Perkins that most effectively addresses skin laxity without needles or surgery. Total Tightening combines ReFirme™ and Titan into one treatment to synergistically heat the entire upper half of the dermis. A series of 4 treatments is recommended for best results. Total Tightening is exclusive to Evolutions Medical Spa Santa Barbara!
Part 2: Skin Lightening – Improving Excess Pigmentation

What is Pigmentation?

Pigmentation is melanin in the skin. Melanin is produced by melanocytes and different skin types have differing levels of melanocyte activity.

melanin Why Does the Body Produce Melanin?

Every body has a normal production of melanin. The body also produces melanin in response to sun exposure (suntan = skin injury), trauma, and inflammation. Melanin production can be accentuated by hormones, birth control pills, and pregnancy.

Pigmentation Problems

  • If melanocytes are over stimulated, appearance problems can occur and can be commonly referred to as sun spots, age spots, “liver” spots, and freckles.
  • Post Inflammatory Hyperpigmentation (PIH) can cause darkening and discoloration as a result of an inflammatory reaction, or injury, to the skin. Common causes are scratches, burns, cuts, bruises or even certain rashes.
  • Melasma, darkening patches of the skin, usually appears on the face (although it can develop on the forearms and neck) and can be triggered by pregnancy, birth control pills, photosensitive drugs, sun exposure.

Treatment for Hyper Pigmentation

Sunscreen: Proactively protect your facial and chest skin year round using a good sun block every day.

Home Care: There are products that can be used at home to fade pigmentation (bleaching agents) and increase cell turnover.

  • Bleaching agents: Recommended bleaching agents are the Obagi Clear, which is Step 3 in the Obagi Nu-Derm system, Obagi C-RX Night Cream, both contain Hydroquinone, PCA Brightening Therapy, Nia24 products and retinoids.
  • Increase cell turnover: Alpha-Hydroxy Acids, Amino Fruit Acids and Retinoids all play a roll in expediting cell turnover.

Chemical Peels: Chemical exfoliation, which is what happens when a chemical peel is done, helps to achieve / maintain clear and youthful skin, especially because as we age, the natural exfoliation process tends to slow down, leaving skin dry and dull. Evolutions offers three types of chemical peels:

  • Phase Five Peel (light peel): low irritation and not a lot of actual peeling – a series of five is recommended.
  • Alpha Beta Peel (medium peel): a more aggressive medium-depth peel that stimulates new collagen and elastin. Clients usually see results in as little as four to five days.
  • Obagi Blue Peel (deep peel): the most aggressive peel that will significantly improve the complexion, leaving the skin firmer, clearer, smoother and healthier. Particularly good for pigmentation issues too.

Vibradermabrasion: a physical exfoliation to stimulate collagen. Vibraderm keeps your skin looking young and can be used to treat superficial pigmentation problems (with no downtime) and, in addition, improves penetration of products / peels. It is superior to microdermabrasion because there is no suction, which can cause broken capillaries.

Light-based Treatments: The FotoFacial RF is the “gold standard” for treatment of pigmentation. It works deeper than most chemical peels and is available in a series or combination with other treatments (FotoPeel, FotoPeel Plus, and Foto Genesis Packages).

Sublative Rejuvenation: The newest fractional resurfacing device available that is used to treat wrinkles, pigment, acne scarring, skin laxity, tone and texture that uses radiofrequency energy instead of light, so it is safe for all skin types. Less downtime and discomfort than older fractional laser devices, with customizable treatments available. Sublative Rejuvenation can be combined with other light-based treatments such as the FotoFacial RF or Total Tightening.

Chemabrasion: a surgical procedure that combines a deep peel with dermabrasion with approximately a one-week downtime. Chemabrasion treats wrinkles, sun damage and pigmentation issues.

In conclusion, Dr. Perkins stressed that the best way to treat sun damage is to prevent it – use sunscreen daily, year round. When pigment issues arise, however, there are a number of treatment options – from “at home” care to treatments at Evolutions.

The supplements come in powder form, making it easy to take with you to work, school and vacation and can be mixed with 6-8 ounces of water for a healthy drink.
