Botox use is at the top of the charts, again! The American Society of Plastic Surgeons released their stats for cosmetic surgery and minimally-invasive procedures in 2014. Overall aesthetic procedures, such as those offered in our Santa Barbara medical spa, are on the rise. Here are the top 5 minimally-invasive treatments performed in 2014:
There were almost 14,000,000 minimally-invasive cosmetic procedures done in 2014. We believe that these treatments are so popular because they yield great results with little (or no) downtime and are affordable to most.
For those of you who are not as familiar with dermal fillers, Sculptra, and/or the wrinkle-reducers Botox and Dysport, we will happily share with you some information about each of them. Perhaps it will help you to make a decision about getting an injectable treatment or encourage you to add a new treatment to your anti-aging arsenal.
Botox and Dysport Botox and Dysport treatments are one of our most popular services at Evolutions, year round. This is not surprising! In 2012, there were over 6 million Botox or Dysport treatments in the US. Botox and Dysport are forms of botulinum toxin type A. They are both injected under the skin to treat overactive muscles in the forehead area. Botox and Dysport block the signals to the muscle that tell them to contract, resulting in a smooth, frown-less forehead. Botox or Dysport can take from 1 to 7 days to “set in” and start working. Both are FDA approved and are deemed safe and effective in the treatment of facial wrinkles. Dysport is a smaller molecule than Botox, so the units are different (i.e. 20 units of Botox equal 60 units of Dysport). Results are temporary and last about 3 to 4 months.
Restylane and Perlane: Dermal Fillers Restylane and Perlane are in the same family of products. They are both dermal fillers made from hyaluronic acid that are formulated to act like your body’s own naturally produced hyaluronic acid. These are injected under the skin to fill in moderate to severe facial wrinkles and folds. Restylane can also be used to add fullness to the lips. Replacing volume lost in the face can help restore a more youthful appearance. Perlane differs from Restylane in the average size of the particles and the depth of the injection. Your Evolutions clinician will help you determine which product is best for you. Results are instantaneous and can last 6 months or more (in most cases).
Radiesse Dermal Filler Radiesse is also a dermal filler. It differs from Restylane or Perlane in that it is comprised of calcium microspheres in gel. Radiesse is also injected under the skin to provide volume and structure in areas where volume has diminished. The bonus that Radiesse offers is that it works to stimulate your body to produce it’s own natural collagen. As the calcium gel is absorbed, your own natural collagen will remain behind, giving you volume and structure. Results are instantaneous and have been clinically proven to last one year or more.
Sculptra Aesthetic Our newest treatment, Sculptra Aesthetic, is a bit different than hyaluronic dermal fillers or Radiesse. This new facial injectable does not give instantaneous results, instead subtle results are achieved over time. Sculptra is made from poly-L-lactic acid, a biocompatible, biodegradable synthetic material that has been used by physicians for years in dissolvable stitches. Sculptra works deep within the dermis to gradually restore the collagen foundation that is lost as we age. With a more solid foundation, fullness is steadily returned to facial wrinkles. Results can last two years or more. Most clients require a full treatment of Sculptra Aesthetic, which is an average of three injection sessions over a few months, for the best outcome. Full results are gradually seen over 3 to 6 months.

Santa Barbara cosmetic surgeon, Dr. Terry Perkins, and founder of Evolutions Medical & Day Spa, has been an advocate of the use of Botox before many of us even knew about it. In fact, Dr. Perkins was one of 16 doctors selected by the FDA to conduct a study in 1999 of Botox which led to FDA approval of Botox for cosmetic use. Since then Botox has become a very popular treatment for those who want to “erase” their frown and wrinkle lines from their foreheads. However, the latest exciting news is that Botox may also be beneficial in keeping wrinkles at bay. Repeated weakening of the muscles over time can help prevent the wrinkle(s) from forming or from becoming too deep.
To further support this, a study was done on identical twin sisters to evaluate the long term effects of Botox use. One twin received Botox approximately two to three times per year in the forehead and glabellar region (frown lines between the eyebrows) for 13 years. The other twin received Botox only twice over the 13 year study (at years 3 and 7). The results showed that imprinted forehead and glabellar lines were not evident in the twin that was regularly treated with Botox but were evident in the twin that was only treated twice. It was also noted that untreated facial areas (the nasolabial folds – the line that forms from the nose to the corner of the mouth) showed comparable aging in both twins.
Recently an article that was published on “Today Health” cited the twin’s study and also referred to an article by JAMA Dermatology which stated that it is “rarely too early” to start the “conservative and thoughtful use of neuromodulators [i.e., Botox], fillers and noninvasive energy-based treatments.” The article goes on to say that it is not endorsing the use of Botox in children or teens, nor are we, and even the 20s may be too early as well. Signs of aging varies by individual and often will depend on many factors, including a person’s skin type, their genes, and any environmental damage (sun and UV exposure), but normally begins in the 30s for most people.
The best time to start Botox as a preventative treatment to help inhibit the progression of deeper wrinkles would be at the onset of wrinkles (even if they are very mild). We often get asked what the difference is between “fillers” and Botox. Botox is a purified protein that is used to treat overactive muscles, temporarily relaxing them to smooth out the wrinkles. Botox is used in the upper face to most commonly treat (in order) the frown lines, crow’s feet, and the forehead. It can be used in other areas of the face and in the neck, as well. Botox also effectively treats hyperhidrosis – excessive sweating – in the hands, feet, and armpits.
Dermal fillers are used to restore volume that is lost in the face as the aging process occurs. Dermal fillers are more commonly used in the lower face – in the cheeks, nasolabial folds, the nose, lips, pre and post-jowl, and the corners of the mouth. Fillers are also used in the brows, temporal and central forehead as well as around the eyes. Dermal fillers are temporary and last between six and twelve (or more) months depending on the brand of filler used. Prevelle Silk lasts the shortest amount of time and Radiesse generally lasts the longest.
Recently we ran across an article in NewBeauty.com that speaks about why facial volume matters and what can be done to restore fullness, either temporarily or permanently, to achieve smoother, younger-looking skin. The article states that we begin to lose facial fat “as early as our 20s, and it’s a downhill slide” from there. It goes on to say that “facial fat deteriorates with age, but weight loss and hormonal changes can speed up the process” as well. When this happens, we are left with sunken cheeks, a down-turned mouth, hallow eyes and a soft jaw. Dermal fillers, such as Radiesse, Juvederm, Perlane, Restylane, or Prevelle Silk can help “fix” these problem areas by plumping the skin, creating more firmness and definition in the face.
