Chemical Peels: What They Are and What They Can Do For You


Chemical peels are “the best bang for your buck” because they provide a great exfoliation in a short amount of time that usually results in dramatic changes to the skin (dependent upon the strength of the peel: stronger peels yield more noticeable results). In general, besides the immediate results of smoother skin and removal of environmental damage, chemical peels also help keep you ahead of the aging process.
How Skin Turnover Works
Basic facts about skin turnover helps to understand how chemical peels work. Skin cells are created in the basal layer and move upward. Cells die as they get closer to the skin and these surface cells fall off, to be replaced by new cells. In young people, a normal turnover is about 28 days, but as we age, the rate decreases to 40 or more days. Along with the decrease in skin turnover, the collagen and elastin in the skin decreases, combined with sun damage, and the result is thinner, less plump skin.
What Do Chemical Peels Do?
Chemical peels expedite cell turnover through exfoliation and stimulate new collagen to thicken the dermal layer. New cells replace the exfoliated cells at a faster rate and collagen and elastin production is increased.


The Benefits of a Chemical Peel
In the big picture, regular peels can create healthier, younger-looking skin throughout your lifetime. They can smooth the skin and improve texture and color uniformity. Peels will also:
  • Lighten by getting rid of superficial damage (age spot, sun spots)
  • Tighten by increasing collagen and elastin
  • Brighten by getting rid of the dead cells on the surface that build up (especially when cell turnover slows down with age) and creating plumper looking skin.
Question from the audience: Would peels get rid of freckles?
Answer from Dr. Perkins: Freckles are typically deeper down and peels would not get rid of freckles. If you are only trying to get rid of freckles, a peel would not be the right choice.

Do Chemical Peels Hurt?

The mild peels have a warm, tingly feeling and may create a minor discomfort. For the medium and darker peels, Evolutions has cooling devices to help make a peel more tolerable.
Will Skin Peel Off?
Yes, the superficial layer does peel off but since the peel is a “controlled injury” to the skin it does not produce scarring. Some lighter peels, such as the AFA Peel, creates a micro-exfoliation that may have very minor peeling such as flaky skin, but still gives you the benefit of a peel.
Question from the audience: When do you see peeling?
Answer from Dr. Perkins: In medium to deeper peels, the peeling may begin anywhere from 2 to 5 days post treatment.


Where Can Peels Be Done?
Realistically peels can be done anywhere on the body. Generally it is okay to be more aggressive on the face, but below the jawline you want to be more conservative.
Are Peels Mainly Done for Aging?
No, peels can be done for skin with acne and hyperpigmentation issues.
Are Peels as Effective as Laser Treatments?
Peels and laser treatments do different things. Laser energy can go deeper than peels can. Chemical peels are more reasonable to do cost-wise and are often a good starting point to get rid of superficial damage. For deeper damage, laser treatment may be considered. Note that Evolutions does offer a FotoPeel series that combines a FotoFacial using Intense Pulsed Light (IPL) and RadioFrequency energy to help with blood vessels, brown spots and more. These are alternated with chemical peels to get the maximum benefit from both treatments.
Question from the audience: Do peels help with large pores?
Answer from Dr. Perkins: Not necessarily. Pore are better treated with Laser Genesis that heats the collagen to tighten the pores.

Pumpkin Antioxidant Facial

  • this peel combines the AFA peel with a clay mask to absorb oil and skin impurities
  • called the “Advanced Acne Facial” because not a lot of peeling occurs

  • strength: light to medium

More Aggressive Options
Dr. Perkins offers more aggressive options at his Cosmetic Surgery Center in Santa Barbara (located near Cottage Hospital). Phenol peels and chemabrasion are done in the operating room and can be great for acne issues, scarring, deep wrinkles. The change is dramatic and there is more downtime than a peel done at Evolutions.
How Can You Improve Chemical Peel Results?

Santa Barbara Medical Spa Event Recap: Chemical Peels, FotoFacials and More

Evolutions Medical Spa’s Educational Event last week addressed skin pigmentation and how to “correct” hyperpigmentation concerns. If you have pigmentation concerns, there is no better time than now to address this! What is Pigmentation?

  • Melanin in the skin causes pigmentation – that varies by an individual’s skin color (darker skin has more melanin, lighter skin has less).
  • Melanin is produced for protection in response to sun exposure (for example, a sun tan is really an “injury” to the skin), trauma, inflammation.
  • Melanin can be influenced by hormones, birth control pills and pregnancy (the “mask of pregnancy” often appears as melasma, darker spots, on the cheeks).
  • What if Melanocytes are Over-Stimulated?

  • Appearance problems of the skin occur when melanocytes are over-stimulated. This hyperpigmentation is commonly referred to as brown spots, age spots, liver spots, and/or sun damage.
  • Post-inflammatory hyperpigmentation can also occur from pimples.
  • Melasma – a broad patch of pigmentation – can occur and can be in the epidermal or dermal layers of the skin.
  • Epidermal pigmentation is more readily treated because of the location – the outer-most layer of the skin. Dermal region pigmentation can be more stubborn to treat. At Evolutions, we use a Wood’s Lamp – a diagnostic dermatology tool – to look at a client’s hyperpigmentation location to find a treatment best suited for them.

    Hyperpigmentation Prevention: Use Your Sunblock Daily!

    Before we address pigmentation correction treatment options, we want to stress that prevention is key! Santa Barbara cosmetic surgeon Dr. Terry Perkins recommends using a sunblock that is at least SPF 30 and is a physical block – one that contains titanium dioxide and/or zinc oxide. Physical sunblocks reflect the sun’s energy whereas chemical sunscreens heat the skin up.

    Dr. Perkins and the Evolutions staff recommend to clients a highly effective sunblock called TIZO3 SPF 40. TIZO is free of paraben, oil, PABA and fragrance. It gives the skin superb protection while leaving the skin with a silky, smooth, matte finish. TIZO products are available at Evolutions.

    Hyperpigmentation Treatment Options

  • Home Care: There are many skin lightening agents on the market that are available at your local drugstore or through a physician’s office. Dr. Perkins recommends the Obagi Nu-Derm or Obagi-C Rx programs, PCA Brightening Therapy, NIA 24, and retinoids (also known as “Retin A” or “tretinoin”). Another home care option, maybe in addition to a skin brightening/lightening agent, is to increase cell turnover through products with alpha hydroxy acids (AHA), amino fruit acids, and, again, retinoids.
  • Chemical Peels: a chemical peel is a “controlled” injury to the skin that creates enough injury to cause an exfoliation. Evolutions offers chemical peels in different strengths, depending on your overall goal and the amount of downtime you may have to dedicate to the process.   • Vibraderm: a superficial exfoliation using abrasive aluminum paddles that provides a gentle abrasion that is faster than microdermabrasion and does not have a vacuum component (which can cause broken capillaries). Great in a series or with in conjunction with other procedures as it will improve the penetration of treatment products and chemical peels.
  • FotoFacial RF™: an Intense Pulsed Light (IPL) treatment with a broad spectrum of light that gives the FotoFacial RF™ the ability to treat different parts of the skin as the light is absorbed at different levels. The FotoFacial RF™ significantly improves your skin texture and appearance by reducing redness or rosacea, age spots, sun damage, spider veins, fine lines, uneven pigmentation and large pores. It is not a treatment for wrinkles. Your skin may be pink on the day of treatment but other than that, there is little down time.
  • Sublative Rejuvenation: the newest fractional resurfacing device on the market that uses radiofrequency and can treat any skin color. It can treat mild to moderate wrinkles, brown spots, and can improve the appearance of acne scars and textural irregularities. There is less discomfort with this newer treatment in comparison to the older Matrix RF treatment and the down time is not as long.
  • Chemabrasion: this treatment is done at Cosmetic Surgery Center by Dr. Perkins. It combines a mid-dermis peel with dermabrasion. For more information on chemabrasion, set up a consultation with Dr. Perkins at Cosmetic Surgery Center, 805-563-0000.

    How do chemical peels work and what do they do?

    The Obagi Blue Peel is the deepest, most aggressive peel. It is excellent for pigmentation issues and will significantly improve your complexion – resulting in tighter, firmer, smoother, more youthful-looking skin. For optimum results, a pre- and post home care kit is recommended. The Obagi Blue Peel is only available through physicians.

    Chemical Peels 101: Chemical Peels Offered at Evolutions Medical Spa Santa Barbara

    Evolutions Medical Spa offers mild, medium and deep peels as single treatments and also as series. Two of the most common types of peels used are: Alphahydroxy acids (AHA’s) and Trichloroacetic acid (TCA).

    Alphahydroxy Acid Chemical Peels (“Mild” Peel)
    Alphahydroxy acids (AHAs), such as glycolic, lactic, or fruit acids are the mildest of the peel formulas and produce light peels. These types of peels can provide smoother, brighter-looking skin for people who can’t spare the time to recover from a TCA peel.

    With a mild AHA peel, you probably won’t peel at all, but, only get some temporary redness and/or dryness. And the process that you experience will not prevent you from working or engaging in your normal activities. Improvements from AHA peels may be very subtle at first. You may detect a healthier glow to your skin. With continued treatments, you will notice a general improvement in the texture of your skin.

    Trichloroacetic Acid (TCA) Chemical Peels (“Medium” Peel)
    Trichloroacetic acid (TCA) can be used in many concentrations, but it is most commonly used for medium to deeper depth peeling. With a medium strength TCA peel, you could actually go out to dinner that night (with a little makeup). You might be a little red and tight, but, no one would know that you just had a peel solution applied. In about 1-2 days you will start the peeling process. The down-time after treatment is usually 3-4 days of peeling.     Obagi Blue Peel (“Deep” Peel)
    If you want to dramatically improve your complexion in about 2 weeks, go for the Obagi Blue Peel (“Blue for a day!”). The blue solution allows the clinician to control the evenness of the peel; while the number of coats influences the depth of the peel. Immediately after the procedure, the bluish tint remains on your skin, which washes off in the first few hours. In about a week to ten days, your new skin will be apparent and you should be healed sufficiently to return to your normal activities. During this time, it is especially important that you use a sunblock or blotchy, irregular skin coloring may result.

    What to Expect During a Peel
    During a chemical peel, most patients experience a warm to hot sensation that may last about 5 to 10 minutes and may be followed by some stinging. At Evolutions we have a Synercool machine that blows cool air on your skin to ease the stinging. 3-4 layers may be applied and the solution is then neutralized. The whole process only takes about 45 minutes.

    Looking Good!
    The results are not permanent, as a chemical peel cannot stop the skin from continuing to age. Depending on the strength of the peel, periodic treatments may be necessary until the desired effects are achieved.

    Because sun exposure accelerates signs of aging, protecting the skin from sun exposure will lengthen the amount of time that the results are seen. Protecting the skin from the sun also can prevent some signs of aging. Ask your clinician to recommend a sunblock with adequate UVA and UVB protection and use it every day.

    One of our favorite sunblock products is TIZO3. It has an SPF of 40, is water resistant, and has a slight tint to it. The Tizo3 has a velvety, non-greasy feel and gives you a little coverage too. Also, Nia24 with 5% Pro-Niacin®, not only provides SPF30 UVA/UVB protection, but, also repairs past damage and promotes skin health.

    It is very important that you find a clinician who has adequate training and experience in chemical peels. Permanent skin lightening and lines of demarcation may occur, if not done by a well trained medical aesthetician. Cosmetic surgeon Dr. Terry Perkins, who is also the owner of the Cosmetic Surgery Center Santa Barbara, personally sets protocals, trains and supervises all procedures at Evolutions.
